Discussing goals of care
How to navigate family dynamics
Strategizing and problem-solving related to personal concerns and obtaining caregiver support
Communicating on your behalf to care teams at residential communities.
Clarifying each option available such as the differences between memory care versus assisted living.
Learning your loved one's appropriate level of care.
Locating and reviewing quality resources that may help keep you in your home longer.
Interviewing, establishing and coordinating a personalized assistance team (certified nursing aides, home support staff, home maintenance services, lifestyle or domestic assistance or other professionals).
Educating your current caregiver or family member on the best practices for managing common challenges or behaviors associated with cognitive impairments.

Fees:

$95 per hour for in-person sessions, with a 2 hour minimum.

Service plan attendance: $95 per meeting (flat rate)

Administrative fee (off-site research, interviews or phone calls): $85 per hour

Travel fee charged at standard IRS rate per mile.
